To me, it seems that the extremely strong urine smell on the floor tiles in the living area attracts this puppy to use the tiles as toilet area. The use of brand new newspapers does not help.

Neutralise all smells using vinegar:water 1:3 or commercial products.
Use the 2nd sheet of the soiled newspapers.



As for bedding, enclose the crate. Puppies usually feel more secure sleeping inside an enclosed crate. Put towel and feed inside. Since she finishes all food in 10 minutes, she may associate the crate with good vibrations and sleep there. Some privacy too as the puppy may need some rest from the children.
